Induction Process

Please complete the following forms and readings to conclude your induction process.

1. Induction Manual - please read and sign off with Admin
2. Code of Conduct - please read and sign off with Admin
3. Health and Safety - please read and sign off with Admin
4. Personal Information Form - please complete the online form
5. IR330 Form (tax code) - please complete and return to Admin at info@mauao.nz
6. KS2 Form (Kiwisaver) - please complete and return to Admin at info@mauao.nz
7. Police Vetting - for those working with children, please read page 3, and complete pages 4 and 5
8. Please email a copy of TWO forms of ID to Admin at info@mauao.nz - ensure one is photo ID, such as a driver license or passport
